Create/Modify/Terminate Account

**  Account Request forms must be submitted by either the Clinic Manager or a designated department representative **

Health Information Technology provides access to network resources and clinical applications for Clinicians/Faculty/Staff of the organization. This includes Windows accounts with access to email services, Citrix, Centricity Business, Centricity Practice Solution (CPS/EMR) and Radiology Information Systems (ARIS/PowerScribe/PACS). 

To maintain the highest quality of patient care and provide effective treatment, customers will only be granted access to the clinical and academic systems required to perform job duties.

Please choose the appropriate form to request a new account with clinical/academic application(s) access, modify an existing account, or to terminate an account.  You will need to do the following:

  • Click on the applicable form (listed under Files on the right hand side of this page)
  • Fill it out electronically
  • Submit the completed form to accounts@hc.msu.edu

**Please limit this to one account request per submission**
